This is a powerful AC shaded pole, Bomax, Model 174926, single RPM, standard, replacement, 117 VAC, 60 Hz, asynchronous induction motor with an integrated transformer winding. The motor is built to operate continously at a 117 VAC line voltage. It has a two wires main connection. The extra terminals may be useable like a transformer to power some additional control or other circuit. For a replacement, show-and-tell, or for some projects, this may be a nice motor to have.
